- Promotes community programs on specific health topics

Diabetes Prevention Program Flyer
Prevent T2 is a CDC-approved 12-month program where eligible participants work with a trained Lifestyle Coach to learn healthy eating habits, increase physical activity, and manage stress and problem-solving. There is a $25 fee for enrollment. To be eligible, you must:
- Be 18 years or older
- Have a body mass index (BMI) of 25 or higher (23 or higher if Asian American).
- Not be previously diagnosed with type 1 or type 2 diabetes
- Not be pregnant
And have at least 1 of the following requirements:
- Hemoglobin A1C: 5.7–6.4%.
- Fasting plasma glucose: 100–125 mg/dL.
- Two-hour plasma glucose (after a 75 g glucose load): 140–199 mg/dL.
- Be previously diagnosed with gestational diabetes (diabetes during pregnancy).
- Received a high-risk result (score of 5 or higher) on the Prediabetes Risk Test.

Med-South Lifestyle Program Flyer
A 10-month nutrition program that fuses southern cuisine with the Mediterranean Diet and encourages those to make healthy food swaps. The program is delivered one-on-one with a trained Peer Counselor. There is a $10 fee for enrollment. Eligibility includes:
- Must be 18 years or older
- No previous diagnosis of kidney disease

Childbirth Education (Lamaze) Program
The Childbirth Education Program is available to all expectant mothers within the second half of pregnancy (at least 27 weeks). It is designed to help understand the changes experienced during pregnancy and to prepare for the labor and delivery experience and the postpartum period—classes offered in English and Spanish. CATCH My Breath Program & Tobacco Cessation Education
The CATCH My Breath Program is an evidence-based youth vaping prevention program helping youth make informed decisions about the use of e-cigarettes.
