Care Management

  • For At Risk Children (CMARC) for those birth to 5 years of age identified with special health care needs, exposed to toxic stress, or referred by the medical home
  • Includes home visits
  • No income requirements
  • Children must have Medicaid
  • Must be Lenoir County resident
  • For High Risk Pregnancy (CMHRP) for Medicaid patients at risk for poor birth outcomes
  • Assist with provider’s plan of care and link clients with services and resources
  • Must be a Lenoir County resident
